A Case of Perifolliculitis Capitis Abscedens et Suffodiens

Abstract
We presented a case of perifolliculitis capitis abscedens et suffodiens associated with acne conglobata in 40-year-old male. He had a sausage-shaped plaque containing many fistular tracts on right temporal area and cicatrizing alopecia on occiput. On. the middle portion of upper back he had a adult fist-sized plaque containing many fistular tracts.
Histopathologically epidermis showed mild hyperkeratosis and keratotic plugging and in the dermis most hair follicles were destroyed and replaced by fibrous tissue but there were moderate cellular infiltration composed of neutrophils, lymphocytes. and histiocytes around the remained hair follicles.
On serum electrophoresis alpha©ü-globulin was elevated and gamma globulin was the upper limit of normal variation. Skin tests with mumps vaccine, dinitrochlorobenzene (DNCB) and old tuberculin disclosed the evidence of decreased cell-mediated immunity(CMI). All these findings suggest that immune mechanism may be concerned with the pathogenesis in this case.
He was treated with the internal administration of antibiotic and corticosteroid and with surgical opening and curettage of fistulas tracts.
